District Nurses work with your Doctor and perform nursing procedures in your home for anyone not able to come to the surgery.
The care connection team works with the Doctor to provide patients of 18 years old and over care in their home. The team will assess and care plan the patient once a referral has been done by the GP.
Midwives will see you during your pregnancy by appointment at Hillingdon Hospital.
Our health visiting teams work with parents, carers and families to promote the health and wellbeing of children from pregnancy until school age. Health Visitors are nurses or midwives who have undertaken additional training in community public health nursing. By supporting parents to learn and develop the skills required to bring up their children, our health visiting teams help give the new generation the best possible start in life.
All children 0-4 years old are under the care of the health visiting service in Hillingdon and are referred to the Health Visiting service by the midwives/hospital maternity teams or your GP.
If you are not already being seen by this service you can self-refer. Please see our service information leaflet on how to make contact.
